MD24 Housecall is an innovative healthcare company meeting the needs of patients in their own environment, usually an assisted living or long-term care facility. Given that more and more physicians are seeking to open their own medical house call practices, our patients needs are surely being met. To lower the overhead costs and focus the spending on the patients, many physicians are open for house call visits only and have no offices. They commonly take appointment requests via phone calls or emails with the overall goal to combine the service of an old-time, small town doctor with the latest technology designed to meet their patients emotional and financial needs. Through low-cost cutting edge MD24plus platform with built-in interoperable Electronic Health Record/Personal Health Record (EHR/PHR), our network of physicians is growing to meet our patients demands.

MD 24 has an advantage over any of our competitors because of our recent alliances with national hospitalist groups and hospital chains, leveraging our patent-pending MD24 platform. Hospitalists will refer patients at discharge directly to MD24 and we in turn will refer the patients to the communities where we have existing physicians to i.e. IL, AL and Group Homes staffed by our physicians and providers network. This will be an excellent source for patients recently discharged from the hospitals and in need of our services immediately following discharge care. We will have an important part in ensuring that these patients through our combined effort with national hospitalist groups and hospitals. Our goal is to reduce the readmission rate (national current rate is 22%) to minimum (MD24’s current trial with Dr. Nguyen alone 0.15%).

We are currently seeking physicians, nurse practitioners and physician’s assistants in the following disciplines: Family Practice, Geriatrics, Psychiatry, Podiatry, Pain Management, Dermatology, Cardiology, Pulmonology, Internal Medicine, and Orthopedics.

Description:
The Physician Assistant (PA) provides health care services to patients under direction and responsibility of Staff Physician or Site Medical Director. The PA uses clinical judgments in conducting health assessments making differential diagnosis. Plans are implemented through independent action, health counseling, and collaboration with other members of the health team. The PA promotes optimum health, prevents illness and injury, and manages acute and chronic health problems in the on-site setting.
Essential Functions:
•Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician
•Performs therapeutic procedures such as injections, immunizations, suturing and wound care, and managing infection
•Develops and implements patient management plans, records progress notes, provides home-going instructions, and assists in provision of continuity of care
•Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, and health maintenance
•Orders, interprets, and evaluates diagnostic tests to identify and assess patients clinical problems and health care needs
•Records physical findings, and formulates plan and prognosis, based on patients condition.
•Provides written home-going instructions
•Collaborates with Physician and other health professionals to prepare comprehensive patient care plan as necessary
•Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures
•Refers patients to Physician for consultation or to specialized health resources for treatment
•Maintains medical records (In some health centers it may be using Electronic Medical Record software)
•Other duties as assigned
