Arizona MENTOR, a partner of The MENTOR Network, offers home and community-based services throughout the Grand Canyon state to children and adults with developmental disabilities or behavioral health needs as well as individuals with brain and spinal cord injuries or other neurological conditions. We offer a range of services - including our innovative Host Home program, as well as other residential living options, in-home support, day programs and behavioral health services.
Description Position Objective :
The Program Coordinator position is a first line supervisory position and is responsible for assisting, supporting and teaching the skills necessary to enable the clients to achieve the activities of daily living in a 24 hour residential setting. The Program Coordinator is also responsible for assuring that each living environment is staffed, clean, and well kept and assuring direct care staff is performing their job duties and responsibilities as assigned.
As a MENTOR employee the Program Coordinator shall present a professional image to both internal and external customers. Communications, whether verbal or written, shall be conducted in a way that represents the high level of professionalism that is expected of all MENTOR employees and companies throughout the United States.
Major Responsibilities :
Individual Treatment and Care Plans (ITCP) and/or Individual Support Plan (ISP) Training
1. Provide formal teaching and support in accordance with individual treatment plans and target behavior plans.
2. Assure all required documentation is complete, legible and written accordingly to policy and standards.
3. Complete assessments and provide input into the development of services, goals and objectives.
4. Assist clients in arranging medical and behavioral services.
5. Assist staff to assure training and daily activities are implemented and documented as scheduled.
6. Provide financial management assistance to clients in the residence, as assigned.
7. Assist in arranging transportation to day programs, medical appointments and other community activities.
8. Assist clients in maintaining contact with their families.
9. Assist clients in accessing available community resources.
Treatment and Monitoring of Medical Concerns
1. Provide direct training to clients regarding health and wellness.
2. Monitor and document health information concerns on a daily basis.
3. Assist clients in medication administration and provide training in order to increase their medication knowledge.
4. Assist clients in scheduling, attending, participating and following recommendations of medical and behavioral health providers.
Human Resource Activities
1. In coordination with agency Human Resources department, interview and hire personnel
2. Assure adequate orientation and in-service is provided for all employees supervised
3. Support, assist and discipline staff as necessary
4. Evaluate all employees supervised with input from MENTOR administration staff
5. Identify, coordinate and monitor training requirements and needs
6. Approve leave requests, as appropriate
Budget Responsibilities
1. Develop and evaluate staff schedules to assure compliance with budgeted hours and minimize the use of overtime.
2. Monitor and control household expenditures
3. Prepare payroll, expense reports and all required documentation related to household operation.
General Duties
1. Assure housekeeping, maintenance, safety and sanitation is within standards.
2. Perform monthly fire drills
3. Maintain lawn, both front and back
4. Complete occupancy reports
5. Assist clients in the purchase of food and necessary household supplies
6. Assure the living environment is clean, well kept and meet MENTOR and licensure standards
7. Schedule and conduct monthly staff meetings
8. Maintain client financial and entitlement eligibility data is accurate and current.
9. Assure client room and board is current
10. Implement, direct and document all programming; deliver individual or group counseling services, as specified in client treatment plans.
11. Attend staff meetings, in-service and training activities as assigned
12. Assist individuals to choose and prepare healthy and nutritious meals, per MENTOR standards and to include special diets as ordered by the physician
13. Assure that all company employee and consumer information is held confidential according to MENTOR policy
14. Assure that clients are free from abuse, neglect and exploitation
15. Assist in the admission, transfer and discharge activities, as assigned
16. Other duties and responsibilities as assigned.
Qualifications Qualifications:
1. High School diploma or GED (Must be at least 21 years old)
2. Combination of post-secondary education in a field related to behavioral health and full time behavioral health work experience totaling at least two years, or four years full time behavioral health work experience.
OR
3. A master's or bachelor's degree in a field related to behavioral health. May also substitute a master's or bachelors degree in a field not related to behavioral health plus one year full time behavioral health experience or an associate's degree in a field not related to behavioral health and two years full time behavioral health work experience.
4. At least three years experience supervising staff providing direct services in a service-oriented environment.
